Role:
As a Hospice Chaplain/Spiritual Counselor at Medcare Hospice, you will provide essential spiritual support to patients and families enrolled in our hospice programs. Additionally, you will offer guidance and spiritual care to our dedicated staff and volunteers, ensuring a holistic approach to end-of-life care.
Qualifications:
· Bachelor’s degree in divinity or related field from an accredited seminary or institution; Master’s degree preferred
· Ordination or commissioning/endorsement from an ordaining body recognized by the Endorser's Conference, Veterans Administration, demonstrating standing with the organization (ordination must be obtained within 12 months of hire)
· Documentation showing good standing with faith group or professional affiliation group with no current or pending ethical charges.
· Completion of one unit of Clinical Pastoral Education (CPE) accredited by the Association for Clinical Pastoral Education, Inc.; or in process within six (6) months of employment/position.
· Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system preferred; proficiency in computer skills required.
· Healthcare experience preferred.
· Mobile Driver - Valid driver's license and automobile insurance per Company policy
· Ability to travel and possess reliable transportation.
· Physical capability to navigate various environments including homes, hospitals, nursing homes, and assisted living facilities.
· Clear communication skills in English, both verbal and written. Fluent in Spanish highly preferred.
Responsibilities:
· Develop spiritual care plans in collaboration with the interdisciplinary team based on patient/family spiritual traditions and assessments.
· Provide appropriate spiritual care and interventions to address care plan issues.
· Implement spiritual care plans through personal contact and coordination with Spiritual Care Volunteers, local clergy, or counselors as needed.
· Monitor and reassess implementation of spiritual care plans as necessary.
· Maintain accurate and timely documentation reflecting care plan goals, assessments, and interventions.
· Act as a liaison between Medcare Hospice and local clergy and congregations, interpreting the hospice mission and programs.
· Attend and actively participate in interdisciplinary team meetings, contributing spiritual care objectives to care plans.
· Provide bereavement support to families, staff and the community.
· Perform additional duties as assigned.

Physical Demands:
· Ability to navigate various physical environments including stairs, narrow spaces, and environments with varying temperatures.
· Endurance to manage the demands of fieldwork in hospice care, including time management and travel.
