We are currently hiring for our hospital client, seeking a dedicated CCDA Chart Abstraction Specialist to join our remote team and play a crucial role in ensuring the accurate and efficient exchange of patient information.
Job Description:
As a CCDA Chart Abstraction Specialist, you will be responsible for extracting and summarizing essential information from patient medical records to create Continuity of Care Documents (CCD). This role requires a keen eye for detail, a thorough understanding of clinical documentation, and the ability to work with various electronic health record (EHR) systems.
Key Responsibilities:
- Data Extraction: Review and extract relevant clinical information from patient charts, including demographics, medications, allergies, clinical notes, lab results, and care plans.
- Data Standardization: Convert extracted information into a structured format according to CCD standards.
- Data Entry: Accurately enter standardized data into EHR systems or CCDA-compliant platforms.
- Quality Assurance: Ensure the accuracy and completeness of abstracted data, conducting regular audits and quality checks.
- Collaboration: Work closely with healthcare providers, IT teams, and other stakeholders to ensure seamless data exchange and address any issues related to data quality.
- Training and Support: Provide training and support to healthcare staff on CCDA standards and best practices for documentation.
Qualifications:
- Education: Bachelor’s degree in Health Information Management, Nursing, or a related field. RHIA, RHIT, or CCS certification preferred.
- Experience: Minimum of 2 years of experience in clinical data abstraction, health information management, or a related field. Experience with CCDA standards and EHR systems is highly desirable.
- Technical Skills: Proficiency in using EHR systems and familiarity with CCDA standards and XML.
- Attention to Detail: Strong attention to detail and accuracy in data extraction and entry.
- Communication Skills: Excellent verbal and written communication skills, with the ability to collaborate effectively with healthcare providers and IT teams.
- Analytical Skills: Strong analytical and problem-solving skills to identify and resolve data quality issues.
- Organizational Skills: Ability to manage multiple tasks and prioritize effectively in a fast-paced environment.
