Are you a compassionate Registered Nurse who believes in client-centered care, health equity, and meeting people where they are? We’re looking for an RN–Intensive Case Manager (RN-ICM) to join our team and help ensure clients receive the comprehensive, holistic support they need to thrive.
Position Overview
The RN–ICM provides individualized, intensive case management services based on each client’s unique healthcare needs. This role follows regularly scheduled office hours as agreed upon with the supervisor, while also requiring some flexibility for client needs outside standard business hours.
Key Responsibilities
The RN–ICM will:
- Conduct Intensive Case Management assessments to identify unmet healthcare needs.
- Collaborate with a client-centered care team—including Medical Case Managers (MCM) and other healthcare providers—to develop strategies that connect clients with appropriate services.
- Provide tailored support based on client need, including:
- Medication management and adherence support (client education, medication schedules, Direct Observation Therapy (DOT), and pharmacy/provider coordination).
- Coordination of and follow-up after surgeries, medical appointments, and other healthcare events.
- Education on disease self-management, co-morbid conditions, and overall wellness.
- Building and maintaining strong relationships with healthcare providers and community partners.
- Conducting wellness checks and home visits.
- Documenting all client interactions and services in electronic systems such as ClientTrack.
Skills, Knowledge, and Abilities
The ideal candidate will:
- Hold a deep belief in the value and dignity of every person, treating all clients with respect, regardless of diagnosis or personal challenges.
- Communicate effectively with clients by adapting to their language, literacy level, and unique needs (use of translation services, nondiscriminatory language, etc.).
- Demonstrate strong understanding of HIV/AIDS and its impact on individuals and communities.
- Be familiar with behavioral health diagnoses and commonly prescribed medications.
- Be flexible, organized, and able to prioritize tasks in a dynamic environment.
- Be willing and able to meet clients at times and locations that are appropriate and realistic for their circumstances.
Job Type: Full-time
Pay: $65,000.00 - $75,000.00 per year
